Authorize the transfer of your employer-sponsored plan, Traditional IRA or Roth IRA directly to Pax World. Do not use this form for a Roth Conversion or Recharacterization.
Traditional IRA & Roth IRA Rollover/Conversion Certification Form
If you have received a check payable to you from an employer-sponsored plan, Traditional IRA or Roth IRA that you wish to roll over to Pax World within 60 days.
Roth IRA Conversion Forms
Transfer & Convert your Traditional IRA to a Pax World Roth IRA or convert your Pax World Traditional IRA to a Pax World Roth IRA.
