You have selected to roll over your current Coverdell Education Savings Account (ESA) into a Pax World Coverdell ESA. Before completing the necessary forms, please read the Prospectus, the Agreement and money Market Account Fact Sheet carefully and then choose one or more of the Pax World investment options.

Option 1:

If you wish to authorize the transfer of your Coverdell ESA directly to a Pax World Coverdell ESA for the same Designated Beneficiary.

If you have received a check payable to you from a Coverdell ESA that you wish to roll over to a Pax World Coverdell ESA within 60 days.

Upon receipt of your initial contribution, Pax World will mail you a confirmation with your account number and the number of shares purchased.
